"Four-tuh" - say it the same way you would say "fourty", except, of course, go "uh" at the end, instead of "ee".
Four fingers up on the left hand, and all digits up on the right.
By the way, by way of review: this is a mix: conventional finger counting on the left hand, and binary finger counting on the right. Originally, I used binary finger counting on both hands - but then I realized that:
a) I did not need to and
b) using conventional finger counting on my left hand minimized finger fatigue
40 dtg (again: "four-tuh") = decimal 128, or "one hundred and twenty eight"
